New Patient Registration
All new patients applying to join the practice will be asked to complete a simple application form (available from reception) and, if accepted by the Practice, you will be given a joining pack which includes:
- New Patient Registration Information and Guidance
- New Patient Registration Form - to complete for registration
- New Patient Information Questionnaire - to complete for registration
- Practice Leaflet
Once we have received the Registration Form and Information Questionnaire the actual registration will be completed. You will be informed of your "Named GP" at this stage. The new patient information questionnaire should be completed in full as this enables us to identify your health status and any special requirements you may have to help you access our services. In addition, you are invited / encouraged to make an appointment for a health check within 6 months of joining the Practice. The health check gives us the opportunity to update your health status and enables us to provide health services and lifestyle advice appropriate to your needs.
Patients refused a place on the list will be informed of the reason in writing.
The practice area is defined and kept under review by the doctors and can be seen at the surgery.
PLEASE NOTE: In order to ensure the quality of services it may be necessary, from time to time, in liaison with Walsall Clinical Commissioning Group, for the Practice to close its list.
